(AB and BC Only) The Follow-Up Tab

The Follow-Up tab displays details for each drug that has been flagged as DTP in the Medication Record tab. All fields are mandatory. Once complete, the form will be read-only.

Entering the DTP Follow-Up Details

  1. From the Medication Record tab, select the DTP checkbox for the specific drug.
  2. Select the Follow-Up tab. The drug details will display at the top-left of the form.
  3. Select the issue from the Drug Therapy Problem (DTP) drop-down list.
  4. Enter details for each of the fields (all are mandatory).

Note: If the follow-up is due to an issue outside of a DTP, it can be added as an issue rather than as a DTP. When the Add Issue button is selected, a text box will open to document the issue.

  1. Select the Action to Resolve Issue and enter the Monitoring Parameters.
  2. Chose when the next follow-up is to be scheduled from the Recommended Follow-Up Schedule drop-down list. The date will auto-populate in the Schedule Date field.
  3. If the medication review is finished, select Complete. The Follow-Up form will become read-only.

Note: After completing the Follow-Up form in the Medication Review App, the corresponding Medication Review prescription will be placed in the Workbench > To Do tab.

  1. Process the prescription in PharmaClik Rx as usual to be reimbursed for the follow-up
